Purpose: Loss of control over eating (LOC) is common among adolescents and is associated with negative physical and psychosocial outcomes. Recent evidence points to a role for poor inhibitory control (IC) in explaining LOC among adults, but evidence in adolescent samples is limited. Moreover, research remains inconclusive on whether these IC deficits are related to food-specific or rather general types of stimuli. Therefore, the aim of the current study was (1) to examine whether adolescents with LOC experience more IC-problems compared to healthy controls and (2) to clarify whether these problems are food-specific or rather general. Method: A community sample of 133 adolescents (63.2% female; M = 13.06 years; SD = 2.02) was recruited. With the use of a clinical interview (Ch-EDE), 30 adolescents reported LOC. To measure IC, both a self-report measure (i.e. BRIEF) and a behavioral measure (i.e. Go/No-Go task) was used. Results: Adolescents with LOC have more self-reported IC-problems compared to healthy controls. However, no differences were found between the two groups using the Go/No-Go task. Conclusions: These results show that IC-problems are an important mechanism in LOC among adolescents. More research is needed to clarify if these problems are food-specific or rather general.

Computer Training of Attention and Inhibition for
Youngsters with Obesity: a Pilot-study.

Obesity is a widespread problem that starts from an early age. Previous studies suggest that obese youngsters have an attentional bias and an automatic approach tendency towards high-calorie food and display difficulties inhibiting impulses, which may result in a higher intake of (high-calorie) food. An interesting idea for improvement of the current obesity treatment is adding a program that enables to train their difficulties. Subjects were 36 youngsters aged 9 to 15 years old from an inpatient treatment program for obesity, randomized over a training group and an active control group. The training consisted of six training sessions with cognitive tasks aimed at enhancing inhibition towards unhealthy food items (with a go/no-go task), as well as decreasing a food approach bias (using an approach/avoidance task) and a food attentional bias (using a dot-probe task). The current study evaluated whether training of these characteristics helps obese youngsters to maintain weight-loss once they return home at the end of their inpatient treatment program. Results on the cognitive performances were investigated during two measurement sessions, spread over 5 weeks while weight evolution was followed over 13 weeks. Results showed that this training program did not have an effect on BMI at follow up. Limitations, possible causes and future directions are discussed.

Long slow learning: balancing reward learning
ability and effortful control in eating disorders.

Background:
Eating habits in eating disorders (ED) probably involve maladaptive reward
processes and self-control issues.
Aims: In
this study, we aimed to assess reward learning in female ED patients compared
to female controls and women with major depressive disorder (MDD) and
investigate the role of effortful control in this process. We hypothesized that
overall, patients with an ED have a normal to high reward learning ability compared
to controls and MDD. However, ED patients with restrictive (r) eating behaviour
will have inhibited reward reactivity due to high EC.
Methods:
We included 64 ED patients (r=22,
binging/purging (bp)=42), 48 MDD and 38 controls. All participants performed a
probabilistic reward learning task. ED patients completed the behavioral
inhibition/activation system scales (BISBAS) and the effortful control scale
(EC).
Results:
Relative to controls and MDD, ED patients showed a high reward learning effect
over the three blocks (One-way ANOVAs block*group effect: F=2.904, p=0.022).
Age was included as covariate in the model. Restrictive subtype ED patients
show diminished reward learning over the first two blocks (F=3.758, p=0.05),
due to a significant difference between restrictive eaters and controls
(t=2.174, Tukey-K (adj)p=0.02). Effortful control score predict reward learning
ability over two blocks in restrictive eaters (R=0.296, F=6,354, p=0.014).
Conclusion:
ED patients show a strong reward learning response. However, in the initial
learning phase, restrictive eaters have a blunted reward learning reaction
compared to controls. This might be due to high self-control mechanisms, since
we found that effortful control scores in this group predict reward leaning ability
in the initial learning phase.
Case report: 11-jarig meisje met Pervasive Refusal Syndroom

Probleemstelling:
Een meisje van 10 jaar met het Pervasive Refusal Syndroom (PRS) wordt verwezen
door het liaisonteam van een universitair ziekenhuis met de vraag naar
behandeling van een ernstige eetproblematiek. Naast de volledige
voedselvermijding is er sprake van ernstig dysfunctioneren op gebied van
communicatie en motoriek.
Methode: Gezien de zeldzaamheid van dit syndroom werd
een doorgedreven literatuuronderzoek gedaan. Aan de hand van een case-report
worden de bevinden uit het literatuuronderzoek getoetst. Er wordt een beschrijving gegeven van het diagnostisch
beeld en de vormgeving van de behandeling.
Conclusie:
Aan de hand van een case report wordt de literatuur rond het PRS beschreven en
getoetst. Hierbij ligt de nadruk op het specifieke behandelaanbod en de daarbij
horende uitdagingen. Tenslotte worden enkele denkpistes voor verdere
behandeling aangereikt.
E-Mental Health bij eetstoornissen en verwante moeilijkheden

Doel: Bij E-Mental health maakt
men gebruik van informatie- en communicatietechnologie om welzijn en
geestelijke gezondheidszorg te ondersteunen of verbeteren. Verder biedt het
voor hulpverleners mogelijkheden voor preventie, diagnostiek, behandeling en
opvolging op lange termijn. Voordelen zijn dat grote groepen bereikt kunnen
worden, met toch ook kans op gepersonaliseerde feedback. Er bestaat nog geen
overzicht van online hulpmogelijkheden bij eetstoornissen en verwante
moeilijkheden.
Methode: Aan de hand van
kwaliteitscriteria als privacy en anonimiteit, evidence based, gratis/betalend,
bruikbaar in therapie en taal werden verscheidene online hulpverleningstools
onderzocht. Het gaat hierbij om eetstoornis herstel sites, eetstoornis herstel
apps, meditatie apps, mental health apps (diverse problemen) en online modules
bij eetstoornissen en verwante moeilijkheden (angst, depressie en suïcide).
Resultaten: Uit het
kwaliteitsonderzoek kunnen we besluiten dat er geen Nederlandstalige
eetstoornis herstel apps zijn, maar het Engelstalig aanbod is sterk: Jourvie,
Recovery Record en Rise Up + Recover. Wat betreft Mental Health Apps bevelen we
in het Nederlands Psymate, BackUp en OnTrackAgain aan en in het Engels
Mindshift, T2 Mood Tracker, What’s Up?, CBT Thougt Record Diary en Wellmind.
Een speciale aanbeveling verdient Onlinedagboek.be, dat kan gebruikt worden als
onderdeel van een therapie en een aantal voordelen vertoont tegenover vb. het mailen
van dagboekfragmenten.
Conclusie: Voornamelijk
Engelstalige online tools zijn beschikbaar voor herstel bij eetstoornissen.
Enkele websites en apps kunnen ingezet worden in therapie of bij onderzoek.
Verder onderzoek dient uit te wijzen wat de inzetbaarheid en werkzaamheid van
deze tools kunnen zijn in Vlaanderen en hoe deze middelen verder verspreid
kunnen worden.
PILOOTSTUDIE: VERSCHILLEN IN AANDACHTS- EN INHIBITIEPROCESSEN VOOR
VOEDSEL EN BEWEGING GERELATEERDE STIMULI EN EMOTIEREGULATIE TUSSEN ANOREXIA
NERVOSA PATIËNTEN EN GEZONDE PROEFPERSONEN.

Introductie
Voorgaand onderzoek heeft reeds
uitgewezen dat individuen die lijden aan anorexia nervosa (AN) een abnormale
verwerking van cognitieve en emotionele informatie vertonen, wat mogelijks een
kwetsbaarheid is voor het ontwikkelen en in stand houden van deze eetstoornis.
Om meer inzicht te krijgen in de werkingsmechanismen deze complexe mentale stoornis,
worden in de huidige studie experimentele paradigma’s getest die mogelijke
verschillen in (1) aandachtsprocessen voor voedsel gerelateerde foto’s, (2)
emotieregulatie van negatieve feedback en (3) responsinhibitie met beloning
kunnen detecteren tussen AN patiënten en gezonde proefpersonen. Het gebruik van
voedsel gerelateerde stimuli verhoogt de ecologische validiteit van de huidige
experimentele paradigma’s.
Methode
25 AN patiënten met het
restrictieve type en 25 gezonde proefpersonen zullen deelnemen aan deze studie.
Bij aanvang zal men verschillende vragenlijsten moeten invullen die peilen naar
depressie, angst, emotieregulatie en piekeren, welke als moderatoren kunnen
gebruikt worden in onze analyses. Vervolgens zal men (in deze volgorde) (1) een
engagement-disengagement taak met voedselstimuli (waarbij oogbewegingen gemeten
worden als maat voor aandachtsprocessen), (2) een cognitieve controle taak met
gemanipuleerde negatieve feedback, en tenslotte een go no go-taak met beloning
(met voedselstimuli) moeten uitvoeren. Na iedere taak zullen een aantal
bijkomende vragenlijsten afgenomen worden die peilen naar ruminatie, piekeren
en stemming. Voor de volledige duur van het experiment zal
hartslagvariabiliteit (HRV) gemeten worden, welke een indicatie is van
emotieregulatie.
Resultaten
Aangezien de data inzameling van
dit onderzoek nog maar recent gestart is, zijn nog geen resultaten beschikbaar.
Op de engagement-disengagement taak verwachten we dat patiënten (in
vergelijking met controle proefpersonen) meer disengagement t.o.v.
voedselstimuli zullen vertonen. Tijdens de cognitieve controle taak zouden AN
patiënten ook een lagere HRV vertonen bij het ontvangen van negatieve feedback.
Op de go no go-taak wordt hogere inhibitorische controle en een kleiner beloningseffect
verwacht bij AN patiënten, voornamelijk bij voedsel gerelateerde stimuli.
